The benefits of improved medical care planning are seamlessly reinforced by regular scheduled preventive chronic care consultations. Committed HealthCube-employed GPs attend participating RACFs in person a minimum of one day a week, more frequently as demand requires.
Participating RACFs may offer the PAC service as a clinical care option to new residents, or existing residents whose traditional GP has retired, moved or become unavailable for whatever reason.
Comprehensive Medical Assessment Service
